Your First Appointment:

We will email you a link to fill out the required paperwork. Please bring with you the following items to your first appointment:

  1. Prescription or referral from your doctor * If you do not have a prescription or referral, most insurance companies will cover 30 days or 10 PT visits.
  2. Insurance card
  3. A list of your current medications
  4. Copies of any diagnostic testing (MRI, CT, XRay) or operative reports

*Please wear comfortable clothing that allows you easily to move and allows us to view the body part that is being treated.

We are considered out-of-network providers for most Aetna, Cigna, and United Care plans. You will have to contact your insurance company and find out your out-of-network benefits and any deductible you might have.

 

If you do not have insurance or have exhausted your insurance benefits, please contact the office and we can let you know the cost of the initial and follow-up visits.

* Copays/ deductibles are due at the time of each visit.
